Required Enrollment Packet & Parent Resources

Required Enrollment Packet

To complete your student’s enrollment at Empower Academy, all families must complete the Required Enrollment Packet. These forms help us provide a safe, supportive, and connected learning environment while ensuring we have the permissions and information needed for your child’s success.

Complete the Required Enrollment Packet here

The enrollment packet includes:

  • Digital Citizenship & Technology Agreement – Outlines expectations for responsible use of school technology and digital resources.
  • Student Handbook Acknowledgement – Confirms that you have reviewed the Empower Academy Student Handbook and understand our school policies and expectations.
  • Photo, Video & Media Release – Provides permission for your student to be included in school photos, videos, the school website, social media, and other school publications.
  • Smyrna Public Library Partnership Form – Allows your student to participate in our partnership with the Smyrna Public Library, giving them access to valuable educational resources and learning opportunities.

Parent Resources

We’ve created a Parent Resources Folder with helpful information to prepare your family for the school year, including the school calendar, handbook, supply lists, forms, and other important resources.
